What exactly is integrated health care?

Integrated health care is an approach defined by a high degree of collaboration and communication among various types of health professionals. What makes integrated health care unique is the sharing of information among team members related to patient care and the establishment of a comprehensive treatment plan to address the biological, psychological and social needs of the patient. The interprofessional health care team includes a diverse group of members (e.g., physicians, nurses, psychiatric and other health professionals), depending on the needs of the patient.

Assessments for drug and alcohol treatment services.
Therapy new patient assessments for those interested in starting talk therapy.

As a courtesy to our patients, we will verify benefits; however, the benefits quoted are not a guarantee of payment. It is the patient’s responsibility to verify and get a thorough understanding of their medical benefits. The patient is responsible for all charges not reimbursed by their insurance carrier including but not limited to coinsurance, deductibles, and any charges associated with collection activity.
